Institute 2015

 

All Los Angeles corps members will attend Institute in Phoenix, Arizona. Institute will take place from June 7, 2015- July 10, 2015. Food, housing, and transportation will be covered for all corps members during Institute.

 

Detailed information, directions and instructions are now available on the Phoenix CM Hub. This is where you will learn more about what is the Phoenix Institute, what to bring to institute, how to pack, dates, times and policies. Please review all information and complete any action steps necessary.

Phoenix Institute Registration Day Information
Phoenix Institute begins on Sunday, June 7th and runs through Friday, July 10th at the Tempe campus of Arizona State University. Registration will take place on Sunday, June 7th from 10:00am to 5:00pm PST in the Sun Devil Fitness Complex at 400 E. Apache Blvd, Tempe, AZ 85281. For further instructions, directions, maps, and what information on items to bring with you to registration, please look at the Phoenix CM Hub, Life and Logistics tab, under Moving In & Out. Plan to arrive at Hassayampa Academic Village at 711 E. Lemon St. and signs/staff members will guide you to Registration Day at the Sun Devil Fitness Complex.
 
Please make every effort to arrive during registration hours on Sunday before 5:00pm. You should only use late registration in extenuating circumstances. If you require late registration, please email phoenixinstitute@teachforamerica.org. 
 

Personal Information Form (PIF)  <DUE by April 17th>.
As institute gets closer, we want to make sure that we are meeting some of your needs. In addition, this information is critical for your dining and residential access this summer at Institute. Please complete the Personal Information Form by April 17th, so we can plan accordingly. An email from LAinfo with your unique password to access the PIF will be sent to you by March 24th. If you have any questions or concerns not covered in this form, or have a change in an answer after submitting, please email us at phoenixinstitute@teachforamerica.org. 

Submit Required Measles, Mumps and Rubella (MMR) Immunization Record for Institute
Action Required by May 1st
 

The Phoenix Institute host site ASU requires all corps members to provide proof of having TWO Measles, Mumps, and Rubella (MMR) vaccinations after 1980. This may be provided in one of two ways:
1.Through a copy of official immunization records from a doctor or educational institute
2.Through an official letter from a doctor verifying presence of the Rubeola titer*
*participants who have received the vaccination and are unable to locate their records may request a physician perform a blood test to check for the presence of Rubeola
 
Please ensure the documents include: full name, date of birth, contact phone number, Teach For America Corps Member (must be stated on the document)
 
Scans of MMR vaccination documents must be sent to TWO places no later than May 1st, 2015 with the file naming convention: Last Name.First Name.Region.TFA.MMR (ex: Ho.Jennifer.Phoenix.TFA.MMR:

  • A copy must be emailed to Arizona State University Campus Health Services at measles@asu.edu

  • A copy must be emailed to mmrphoenixinstitute@gmail.com

  • IMPORTANT: You will be required to submit a vaccination record to LMU at a later date this summer. Please retain a copy for your records.

Institute Early Check In Request <if applicable>

Action Required by April 17th
 As announced, Induction will take place in Los Angeles between Monday, June 1st and Friday, June 5th. All corps members will check-in to Institute on Sunday, June 7th in Phoenix. Early check-in at Phoenix Institute may be available on Saturday, June 6th. However, you must arrive on campus for check in between 10am and 12pm or 3pm and 5pm and get pre-approved by Phoenix Institute. To request early check in, email phoenixinstitute@teachforamerica.org by April 17th. As a reminder, corps members are responsible for their own transportation to both Induction in Los Angeles and Institute in Phoenix. 

Submit family housing inquiry for Phoenix <if applicable>
There will be family housing available at Phoenix Institute for corps members who would like to bring their children to Institute. The Phoenix Institute team is currently negotiating contracts with the university site and would like to get a sense as to how many corps members would be interested in family housing. If you are at all interested in family housing, please email phoenixinstitute@teachforamerica.org. Please know that you will not have to commit to family housing until a later date. By inquiring you are simply asking for more information and indicating your interest. 

Submit an Institute Leave Request (if applicable)
Action due by May 15th
 
Corps members are expected to attend every day of Institute. In rare/emergency situations or extremely compelling personal situations, you may apply for one absence. Permission to miss a day must be granted in advance with the approval of your institute’s Senior Managing Director. If you could like to request a one day absence, submit a Summer Leave Request if you are seeking approval to miss one day of institute. Please note that completing this form is a request, and it is not a guarantee. The Phoenix Institute team will get in touch with you about your request after they review it.
